Does anyone know a website where on earth I can gain a diabetic logbook?

I like the outdated fashoined logbooks. Do any companies send them to you for free?Does anyone know a website where on earth I can gain a diabetic logbook?
i own a booklet from my diabetes consultant but the company that produced it are Novo Nordisk and it was free but i reflect on you have to ask your GP to come by a copy.
Check with the ADA or your doctor.
